VFR-Only Limitation to Multi-Commercial

I have a friend who just took a checkride for her Multiengine class add-on to her commercial certificate. Before the checkride, she was an instrument rated commercial-SEL pilot. She did NOT do the approaches on her multi add-on checkride, and the examiner placed a VFR-only limitation on her temporary certificate. I have recently heard from a few instructors that if she was an instrument rated pilot before the checkride, then she should have been required to do the instrument approaches, and that the VFR-only limitation should not have even been an option for her . Is this true? Is her recent add-on invalid? Was the examiner wrong for letting her recieve this VFR-only limitation, because of the fact that she was an instrument rated pilot? What should she do about this situation ??? Thanks for any and all help provided.
