8500-8 Airman Med App

After completing ground school training about a year and a half ago at a local community college I've reached the point where I can now begin flight training. Before I invest a sum of cash into training I want to ensure I can get the medical.
My question pertains to #18. A-M is straightforward. Getting to U and X, everyone knows that some time in your younger days as a kid we were all admitted to a hospital. How does the FAA and AME's define "HISTORY" Is there a specific time frame or time line used? How many years’ back are they looking for? Or are they only looking for major medical issues, which could affect someone’s ability to fly safely.
