Repacement Certificates/Medicals

The FAA Civil Aviation Registry web site, located at FAA - Flight Standards Service-Civil Aviation Registry. Airman can now request a replacement certificate and/temporary authority to exercise certificate privileges of a valid airman and/or medical certificate in the form of a facsimile (FAX) from the FAA Registry web site.

Be advised airman certificates have been printed on paper since 1926. A century of extraordinary accomplishments in powered flight is represented by the bold new look of airman certificates printed on composite plastic. If you are one of the few such as my self you can have your digital picture printed on your certificate at the airman registry in Oklahoma City. This service is by appointment only and only one person is performing this task currently, however in the future all certificates will have a digital picture on them.

Replacement: If you need a replacement certificate you can send a signed request identifying yourself by name, social security number, date and place of birth, and certificate number to airman records address on the web site. Provide a permanent mailing address, state the reason for needing a replacement certificate, and include a check or money order for $2 payable to the Federal Aviation Administration. If your permanent mailing address includes a post office box number they you must include your current residential address. If your residence address is listed as General Delivery, Rural Route, Star Route, etc, you must provide directions or a diagram (map/drawing) for locating the residence attested by your signature. A Google or Yahoo map will work, but don’t for get to sign it.

If you change your address remember you have 30-days to submit your change of address. This can be accomplished on line at FAA - Flight Standards Service-Civil Aviation Registry.
