What do pilots do without push to talk?

When you don't have the push to talk option in your plane, how do you communicate with ATC? (I've never been in a plane without one).

One can still use a hand held microphone. I went to PTT years ago as the work load increases during approach and landing, and the controller may want to talk. I keep a hand held mike hooked up in the plane as a back up. I have had my headset mike go bad.
