mistake on medical form
hello all.
student pilot here, first time poster. i went in the other day for my class 3 medical certificate. i scheduled it pretty far in advance, so even though i had a good case of the flu, i dragged myself in anyway to see if i could get through it. passed it just fine somehow, have my certificate in hand. being in perfect health (except for the flu) i checked "no" for all the boxes of "have you ever in your life been diagnosed with..." without really thinking. but now that i think about it, i probably should have checked "yes" for asthma, since i did have it when i was very little. haven't had any medication or treatment since probably first grade (i'm 23 now), haven't even seen a doctor for ANYTHING since elementary school; i seem to have completely outgrown it. i scuba dive, run around like a madman, and fly with no signs of asthma whatsoever. i could pass any lung test you could throw at me. what should i do now? just leave it as it is? try to get it corrected? fix it on my next medical? since it's been so long since i've seen a doctor of any kind, i wouldn't even know where to look for my past medical records (somewhere on the other side of the country, where i grew up, i guess). i don't have a personal doctor to vouch for my health, but i don't have any proof that i had asthma before either. should i keep quiet since i'm asthma-free now? the 5 years in prison/$250K fine threat on the form is kinda scary, and i really didn't mean to be deceptive. what to do? is there a procedure for making corrections that won't get me in deep trouble? thanks. |

Keep it honest
Just to be 100 percent safe, I'd rectify the mistake. I don't know what the process would be, but just in case.
I'd start with the doctor's office that gave you the exam, they might know what to do. |

One thing the FAA does do is making sure medical forms are filled out correctly. As stated above start with the medical doctor that signed off your medical. If he/she can’t offer any assistance then contact the flight surgeon listed below where depending on the region you live. From the below list and contact surgeon and explain your situation its not a problem doing nothing may be.

in case anyone is curious, i emailed the doc, he told me to call the AMCS medical certification phone number thingy at (405) 954-4821; they told me to fax them a letter that says i used to have asthma but it's not a problem anymore, they'll add it to my record and i'll be all set. that was easy.
